I've heard this theory on what The Lamb is supposed to be.
The Lamb is supposed to be Isaac's, fully Demonic, Anti-Christ Form.
The reason Isaac's mother tried to murder him was because he was cursed to become The Anti-Christ. In truth though, he locked himself in his chest in a basement and ended up suffocating to death instead. The whole game is supposed to be like what was going on in his mind before he died.
"The Lamb" is Isaac's Full Anti-Christ form, which was what would happen if he did not prevent it.
Also supported by how you face against Isaac himself multiple times for some reason. Such as "It Lives" was mentioned by Edward to be Isaac as a fetus, before he was born.
Isaac as the Heaven True Last Boss is Isaac as an Angel. Since he killed himself before he could fully become the Anti-Christ, he was redeemed and became a great angel after that.
And the Heaven True True Last Boss, "??????" Is supposed to be Isaac's corpse, post-mortem.
